Hospice at Home aims to provide personal, practical and emotional support, to enable people to stay at home right up to the end of their life.

By putting people and their families at the very centre of their care, Hospice at Home is designed to deliver holistic person-centred care.

Hospice at Home aims to do this through:   

  • taking a flexible approach, ensuring people have choice in relation to when we visit and what the team do when they are in their home 
  • Providing personal care and emotional support to those who are dying at home
  • Listening to what’s important, to support and empower people and their families to discuss decisions about their end-of-life care
  • Support people to come out of hospital to die at home where this is their preference
  • Support people to remain at home to die and avoid hospital admission where this is their choice 
  • Giving carers time to have a moment of respite  
  • Providing additional services supported by our Complementary Therapy team, such as aromatherapy, massage and relaxation  
  • providing support with memory work with input when needed from our Creative Arts team, which is all guided by the individual’s wishes and preferences.
  • Referring to wider hospice services as needed.

The Hospice at Home Team work closely with District Nursing teams, Social Services and the wider Hospice team such as Clinical Nurse Specialists and Allied Health Professionals to provide care and support for people at home.

How to Refer:

Referrals to the service can be made by any Health and Social Care professional if the individual and their family are agreeable to having this support in their home. The Lead Nurse and coordinator for Hospice at Home will co-ordinate a team comprised of a Registered Nurse and Health Care Assistants who are expert in providing care for people in the last weeks of life.
Health and Social Care professionals can make referrals via telephone on 01324 826222 or 07881 511517 between 9am and 5pm, 365 days per year.  Or by completing the Referral Form and emailing it to [email protected]. In some cases, we may be able to offer support on the day of referral.

    Service Quality

    The quality of our Hospice at Home service is regulated by the Care Inspectorate, who formally inspect the service every year.  They have graded the service as excellent (their highest grade) for the last 3 years.  A copy of the most recent inspection report is available by clicking the link below.

    The Hospice at Home Service has also been externally evaluated and, as well as confirming that the quality of care is excellent, the Report demonstrates that Hospice at Home successfully reduces the amount of time that a patient spends in hospital in the last two weeks of life.
